(1) Please provide a list of all Public Tenders awarded and completed (for your department or agencies from 1 July 2023 to 1 March 2025), in which the actual contract value was 20% higher than the original contract value? Include in the list for each over 20%: (a) The tender code; (b) The tender description for services or goods or both; (c) The region(s) where the tender was actively engaged or completed; (d) Public contact details for the successful tenderer; (e) Original estimated contract value ($); and (f) Final actual contract value ($)? (2) For any completed contracts list, as per 1 (a-f) where conflicts of interest were declared between final tenderer and officer of the WA Public Service? (For any tenders of any estimated value)? (3) Confirm in all cases that the tender procurement documents are 100% coded (in accordance with procurement documentation rules) to a budgeted item, and if not why not: (a) Provide a list of any procurements within your department where a procurement has occurred or purchase order issued in which the budget line item code was not input?
