An internal clinical quality assurance review of service processes at Kalgoorlie Hospital was conducted within the WA Country Health Service.
Root cause analyses (RCAs) are generally conducted following serious clinical events, and in particular sentinel events. Mental health sentinel events fall into two categories: the national definition (which only covers suicide in an inpatient unit), and the state-wide definition (which includes the unexpected death of a mental health client (whether being treated as an inpatient or outpatient). Either of these categories would result in an automatic RCA.
The information arising from the RCAs is protected under AIMS qualified privilege via the Commonwealth
Health Insurance Act 1973.
This legislation prohibits release of this information to third parties.
