I refer to the Commissioner’s Operating Policy and Procedure 10.3: Case Management (Case Officers), Part 4.3, Primary (Initial) Contact Reports, which states that Case Officers shall, within 14 days of being allocated a prisoner, conduct an initial case management interview and complete the Primary (Initial) Contact Report on TOMS, and I ask: (a) For each of the past five financial years, and year-to-date for 2025–26, how many times, upon a Case Officer being allocated to a prisoner: (i) has an initial case management interview not been completed within 14 days; and (ii) has the Primary (Initial) Contact Report on TOMS not been completed within 14 days; (b) Of those instances identified in (a), please advise: (i) the average length of time taken beyond the 14-day requirement to complete the initial case management interview; and (ii) the average length of time taken beyond the 14-day requirement to complete the Primary (Initial) Contact Report; (c) Please provide a breakdown of the data in (a) and (b) by correctional facility; (d) Please advise whether any performance management, disciplinary action, or remedial training has been applied in relation to Case Officers or facilities where the 14-day requirement has not been met; and (e) What measures has the Department implemented to ensure compliance with the 14-day requirement, and what monitoring processes are in place to track ongoing performance?
