refer to the independent review released today into the death of the baby son
of Suzan Al Hulow and Ali Al-Khafaji on 22 March while in the care of a public
hospital run by private health provider Ramsay Health Care.
(1) Can the
minister explain why the family of baby Amir who tragically died were not
included in the review process despite their critical perspective on the events
leading to this outcome?
(2) Who made the decision not to
involve them?

can inform the house why that was not the case. First of all, I extend my
sincere condolences to the family. This is a
terrible outcome of a very wanted pregnancy. The review provided to the family
today was a clinical overview relating to some concerns that the family
had raised from the root-cause analysis. To provide
the chamber with the whole version of events, Joondalup Health Campus
instigated the root-cause analysis, during which the family and all the
clinicians involved were interviewed. The family was very much engaged in the first part of the process. The
RCA was presented to the family and they were provided an opportunity to
respond. Their response was that they had some concerns about some of the
clinical findings in the RCA and they requested a review of the clinical
findings. That review was undertaken by the head of obstetrics at Osborne Park
Hospital. It was communicated to the family at the time that it would be simply
a review of the clinical findings, not a reiteration of the RCA process, so
they would not be interviewed in that process. I understand that this is
extremely difficult to hear, and that it is not what they wanted to hear in the
findings. However, the two reviews found that the clinical care was
appropriate, notwithstanding the fact that the outcome was devastating for the
family—and I acknowledge that.
