(1) What has been the recidivism rate for offenders released from custody in Western Australia in each of the financial years 2021–22, 2022–23, 2023–24, 2024–25, and year-to-date 2025–26, measured by: (a) return to prison within two years of release; and (b) return to custody (including community corrections) within two years of release? (2) For each of the years in (1), please provide a breakdown of recidivism rates by: (a) custodial facility; (b) youth custodial facilities versus adult custodial facilities; and (c) offence category? (3) For each of the years in (1), how many prisoners released on parole re-offended while still under supervision? (4) How many of those re-offences resulted in : (a) return to custody; and (b) community-based sanctions? (5) What has been the recidivism rate among offenders who completed rehabilitation, education, or employment programs in custody (e.g. substance abuse programs, literacy programs, vocational training) compared with those who did not participate, in each of the financial years since 2021–22? (6) What is the current Departmental target for recidivism reduction, and what measures are being implemented to achieve this target? (7) Has the Department undertaken any evaluation or review since 2021–22 into the effectiveness of rehabilitation and re-entry programs in reducing recidivism, and if so, please table the findings?
