(a) I requested a review of cancer services on 15 April 2015 at a meeting with the then Acting Director General (A/DG) of the Department of Health (DoH).
(b)(i) The first report was provided to the then A/DG on 23 June 2015, for the meeting between the A/DG and Chair of the WA Cancer Care Taskforce (the Taskforce) on 7 July 2015.
(ii) The report and its addendum were finalised on 6 October 2015.
(c)(i) Refer to the answer provided to Question on Notice 4531.
(ii) The final report has not yet been discussed between the Director General (DG) and me as the report and its addendum were only finalised on 6 October 2015.
(d)(i) The first report was first provided to me on 9 September 2015.
(ii) The final report and its addendum was provided to my office on 6 October 2015.
