These regulations amend the Health (Notifications by Midwives) Regulations 1994 by replacing Form 2, which is the notification form for cases attended by midwives, including pregnancy and baby details.

Midwives in Western Australia will be required to use the updated Form 2 when notifying the Department of Health about cases they attend. This affects the data collected and the reporting process for midwifery services.
