Tender description

The proposed ‘Integrated self-management and mental health support programs for diabetes’ grant agreement will enable the continued delivery of critical services to people with diabetes living in Western Australia (WA). The self-management programs will focus on building people with type 1 and type 2 diabetes’ ability to self-manage their diabetes. This will include programs on topics such as dose adjustment, carbohydrates and portion control, medication, and foot care. A program will be piloted to provide mental health support via Telehealth for adults living with type 1 and type 2 diabetes.
