An error occurred in the notice published under the above heading on page 5996 of government gazette no. 233 dated 30 december 2016 and is corrected as follows— delete the text of item 3 and in its place insert: “the anaesthetic quality committee established by mark grime, chief executive officer of st john of god bunbury hospital, is declared to be an approved quality improvement committee for the purposes of the health services (quality improvement) act 1994.
