Tender description
The State Party requires the delivery of comprehensive community-based primary health care that is easy to access and tailored to people experiencing or at risk of homelessness. The Service must provide low-barrier, easy to access primary healthcare services consistent with a “ no wrong door ” approach : they are free (or low cost ), conveniently and consistently located, welcoming, and trauma informed . The Service must be designed to address multiple needs in one location or visit, with clear pathways into housing and ongoing supports where required.
